The German DVD label Marketing re-inserted all the deleted scenes of film, which were included in the US-DVD's bonus content, into the cinematic version. Thus, they created a longer version in terms of plot and gore. This X-Rated version is clearly the better choice, even if 2 murders, which can only be seen in the cinematic version, are missing completely. Both versions are featured on the DVD by Marketing.
During the opening credits, there's a tracking shot over a model housing estate.

The warden and Jerry walk down the hallway and through a lattice gate.
There is a different shot in both versions of Jerry ripping out some of his hair to collect it. In the R-Rated, the scene is seen from inside and from close up. The X-Rated only shows the scene from outside, where one can see Jerry from a distance, sitting behind his window.

In this version one now also discovers what happened to the warden. Another guard enters the room with a prisoner and both find the dead warden on the floor. The camera now also zooms in on the corpse, as in the R-Rated. The difference is that here, the shot is from a litte more distance.
The man opens the door of his van. Then one can see him longer while walking to his car.
This scene is only in the R-Rated: The man packs his suitcase into the trunk of his car and says that he bought this car because of its huge trunk. Jerry, standing next to him, slams the door shut with all his might and jams the man's arms, who is howling with pain. In the X-Rated, the scene sets in again when Jerry drives off with the man's car. One can see a piece of his victim's scarf hanging out of the trunk.
Jerry drives down the road longer.
Jerry watches the news on TV. A reporter tells of his escape from the asylum and that he killed two people there. Among other things, the TV also shows how both victims are carried off in body bags.
Here he watches several dating videos of single women. By the look of him, he does not really seem excited about the range of choices.
Jerry, watching a show on a lottery, is seen longer.
Carol also watches the news on TV. The news anchor talks about the killer Jerry Blake, who killed a family and escaped from an asylum where he has killed more people. He says the police was still on the search for him.
Todd enters his room and lies down on his bed. There is a cut to Carol, who sits on the couch and watches the news. Then one sees Todd in bed again, throwing a ball against the wall and catching it. Meanwhile, the news anchor's report can be heard in the background.
The shots of Carol on the couch and Jerry standing outside are longer.
When Jerry walks out of view, the shot is longer.
In the R-Rated, there is now the major part of the scene where Jerry watches the dating videos. The scene could be seen a few minutes earlier in the X-Rated.
When Carol walks out of view, you still see Jerry at the door in the background. This is followed by a change of scene in which Jerry drives down the road in his car.
After Jerry tells Carol that her son was really nice and she thanks him, Jerry smiles in the R-Rated. In the X-Rated he opens the fortune cookie and reads Carol the sentence on the note. Shortly afterwards, both of them laugh.

Jerry apologizes to Carol once again and gets up to go outside to Todd.
Carol tells Jerry that he does not need to take care of Todd so much. Jerry says that he still wants to. The beginning of the next scene is a little longer.
Jerry walks out of the scene. The camera zooms in on Carol's and Todd's picture on the wall. This is followed by a black screen. In return, the starting shot of the therapy group is longer in the R-Rated.

After Matty has left, Jerry walks back into his house and closes the door.
Jerry vents his anger by sawing boards and cursing. This is seen longer in the X-Rated.
In the R-Rated, the abovementioned cut was re-inserted here after a shot of Matty lurking before Jerry's house. In return, one sees Matty walking towards Jerry's house longer in the X-Rated.

Todd walks away longer. Then one sees Phil standing on the other side of the road, gazing after Todd. Afterwards, he goes to Carol's house.
In the R-Rated there is an exterior shot of Matty's house. In the X-Rated one sees a neighbour watering the lawn. Cut to Matty's house. Carol stands at the window, while Matty asks her why her friend Phil sent her to Jerry. After asking the counter question about what would be bad about that, Carol says it was about family counselling.

Jerry, carrying Carol in his arms, is walking up the stairs and they disappear in the bedroom.

Jerry and Carol can be seen longer while having sex. After the climax, Jerry looks over to the alarm clock which shows that it is just after half past seven. After that, there is a change of scene to Matty who is just trying to reach Carol over the phone. She wants to ask her whether Jerry has already confided his dark secret to her. However, nobody answers the phone, so she hangs up with a worried but also annoyed expression and drinks a glass of champagne. This is followed by another scene change. Carol gets up from bed whereupon Jerry wants to know where is headed. Carol says she wanted to take a shower. Jerry jokes about Carol not even having sweated yet. They lock themselves in each other's arms again and kiss. Jerry has another look on the mirror shortly after that.
Matty's death goes off completely different in both versions. In the R-Rated Matty tries to call Carol again. When she hears a sound in the kitchen, she gets frightened and hangs up. She takes the paper knife from the table and cautiously leaves for the kitchen. However, she does not find anyone in there, goes to the back door and locks it a moment later. Suddenly she hears a sound behind her. Terrified, she turns around and finds that her cat has upset the dustbin. Relieved, she goes over to it and takes it in her arm. She leaves the kitchen and takes the cat outside. When she hears steps behind her, she turns around. Jerry approaches with a tautened scarf and puts it around her neck. This is followed by several shots of Jerry strangling Matty to death with the scarf. In the X-Rated, Matty dies differently. Molly gets up and goes over to her gas fireplace. As she bends over a bit, Jerry appears behind her. He grabs her and presses her into the fireplace and turns on the gas tap. After a long death struggle, Molly dies of inhaling gas. Jerry then pulls the body away.

One sees Jerry longer while he is pulling the knife out of his chest + there is a shot of Carol.
There is another shot of the choir in the R-Rated.
Only the R-Rated contains a shot of the choir, with the singers looking away horrified when Carol and Todd, smeared in blood, enter the church.
The end of the R-Rated fades out with Jerry's apparent death. In the X-Rated one can see him lying there longer. Then follows a scene change to the church. Carol has collapsed before the altar and some guests mingle around her and Todd. Finally, there is another change of scene. Carol and Todd are playing with a ball. They then romp about for a bit and walk back to their house. The conclusion of the X-Rated therefore shows a happy ending, whereas in the R-Rated one could only guess how the collapsed Carol was doing.
